Upon entering our office, we will request that you complete our new patient forms. This paperwork will provide us with your health history and information about your condition & concerns. If you have an appointment scheduled, we kindly ask for you to arrive 10 minutes prior to your scheduled time to fill out the New Patient Forms.

Contrary to other forms of Chiropractic offices or clinics, you will not be treated as "another number." Our goal of the initial visit is learn more about you, your condition, and any expectations you may have on how chiropractic care can get you out of pain or simply improve/maintain your quality of life!

 

Depending on your specific condition/complaint, we may take x-rays at this initial visit to have better knowledge on what is going on within your body. The radiographs will help Dr. Nathan come up with the most effective treatment plan, personalized for you. In some cases, the x-rays can catch certain things that we could never see visually. Once your information and x-rays are completed, Dr. Nathan will provide you with a detailed Report of Findings, where he will discuss his recommendations and needs based on your condition and goals. He will then provide the best treatment possible for your needs, once he has your consent.
